Top 40 Under 40 2012 Nomination

Thank you for making a nomination to Avenue's Top 40 Under 40.

Once you have completed this nomination, a second form will be sent to the nominee to fill out. Nominees will not be informed who nominated them.

The second form must be completed by May 10 in order for the nominee to be considered for the 2012 Top 40 Under 40.

This year's list will appear in the November issue of Avenue magazine.
